We Are Going Paperless!
What does this mean to you?
- The forms you complete for us will be done electronically. They will be emailed to you up to 1 week prior to your appointment.
Please look out for an email with the subject:
"URGENT! Nelson Dental Healthcare - Patient Forms"
- You can simply fill out your forms at home and send them back to us securely through our server. All information is encrypted, HIPAA compliant, does not go through email and is sent directly to us. You will be required to set up a User ID & Password that only you have access to. If you forget your password, we will not be able to look it up for you. A link will be emailed to you to reset it.
- If you have sent us your forms in advance, all we will need from you when you come in is your signature.
- If you do not feel comfortable sending your info this way, no problem! Please arrive 10-15 minutes early for your appointment so you can fill out your forms electronically here in the office.
- Once you have filled out your forms electronically, future appointments will only require you to review what you entered the last time and make any necessary changes.
